Sgt. Grzegorz Jakoniuk, 25, of Schiller Park, Ill., died about 7:10 p.m. Iraq time on Nov. 30. Fort Campbell spokesman Cathy Gramling said details on the cause of his death remain under investigation.

Jakoniuk was assigned to B Company, 4th Battalion, 3rd Aviation Regiment, 3rd Infantry Division.

He entered the Army in August 2001 and arrived at Fort Campbell in April 2002.

Jakoniuk's unit is being renamed in a transfer from Fort Campbell to Fort Stewart, Ga., according to a statement from Fort Campbell.

Jakoniuk is survived by his father, Cezary Jakoniuk, of Bialystok, Poland.

Ninety-three Fort Campbell soldiers have died in Operation Iraqi Freedom -- 24 of whom have died in the current deployment that began in September, Gramling said.
